Closing the Hepatitis C care gap—from risk identification to cure.
Breakthrough Health is a recovery-center-embedded navigation model connecting education, testing, treatment, adherence support, and cure confirmation.
Explore the model

The Challenge
A cure exists. The care pathway still fails too many patients.
People with substance-use disorder can be lost between education, screening, confirmatory testing, treatment, adherence, and cure confirmation. The constraint is not the availability of an effective Hepatitis C cure; it is the care-delivery and navigation system around it.
The Model
One connected patient journey.
What I Built
From concept to real-world execution.
- An end-to-end patient journey and clinician/social-worker triage platform.
- A recovery-center pilot and referral pathways grounded in frontline workflow.
- A testing relationship with Boston Health Care for the Homeless Program.
- A manual, non-PHI workflow while compliant infrastructure is being built.
- An outcome-reporting model for public-health and funding partners.
Applied analytics
Decision support—not clinical replacement.
MIT Sloan Analytics Edge work applied logistic regression, CART, and random forest methods to understand how analytics could support triage and care navigation. The model-development work does not replace clinical judgment.
Frontline commitment
One night every week at Hope House.
Saumil personally works with Hope House clients each week, keeping the operating model grounded in the realities patients and staff experience.
Executive signal
“I identify where existing systems fail patients and build new operating models to close those gaps.”
The care-system concept
Designed for the handoffs where care breaks down.
